Postpartum Necrotizing Myositis of the Uterus

چکیده

Postpartum cndometritis is often a mild to moderate infection which responds rapidly to broad spectrum antibiotics. Infrequently, a patient will develop diffuse thrombosis of the vasculature of the uterus,, thereby decreasing tissue perfusion. This results in inadequate levels of antibiotic reaching the myometrium of the uterus. A polymicrobial infection can, therefore, become well established and develop into a synergistic necrotizing infection. Note in (A), the enlarged uterus, the gas present in the uterine myometrium, and necrosis of the lower segment. In (B), the gas pattern is diffusely noted throughout the myometrium. The markedly dilated bowel reflects the presence of intra-abdominal sepsis secondary to infected contents spilling from the disruption of the necrotic lower uterine segment. Antibiotic therapy, clindamycin plus gcntamicin plus ampicillin, was ineffective because adequate tissue levels of antibiotics could neither be achieved nor sustained. A total abdominal hysterectomy was performed and the patient subsequently did well. (C) 1997 Wilcy-I,iss, Inc.

Complete Genome Sequence of Encapsulated Haemophilus influenzae Type f KR494, an Invasive Isolate That Caused Necrotizing Myositis

Haemophilus influenzae serotype f (Hif) is an etiologic agent of bacterial invasive disease. Here, we report the first annotated genome sequence of the Hif strain KR494, which was isolated from a patient suffering from sepsis and necrotizing myositis. The genome sequence will increase the understanding of Hif pathogenesis.

Staphylococcus aureus Panton-Valentine leukocidin targets muscle tissues in a child with myositis and necrotizing fasciitis.

The incidence of myositis has been increasing since the advent of the epidemic of community-associated methicillin-resistant Staphylococcus aureus infection, and Panton-Valentine leukocidin has been implicated as a factor contributing to more-severe muscle injury. We report a case of severe myositis accompanying septic osteomyelitis and necrotizing fasciitis caused by a Panton-Valentine leukoci...

Dermatomyositis and Immune-Mediated Necrotizing Myopathies: A Window on Autoimmunity and Cancer

Autoimmune myopathies (myositides) are strongly associated with malignancy. The link between myositis and cancer, originally noticed by Bohan and Peter in their classification in 1975 (1), has been evidenced by large population-based cohort studies and a recent meta-analysis. The numerous reports of cases in which the clinical course of myositis reflects that of cancer and the short delay betwe...

Management of retained placenta and uterus septum after vaginal delivery: case report

Background: Approximately 3% to 5% of obstetric patients will experience postpartum hemorrhage (PPH). Even though the most common reason for postpartum hemorrhage, as the main cause of maternal death, is uterine atony; other complications such as laceration, hematoma, inversion, rupture; retained tissue or invasive placenta; and coagulopathy may result in PPH. The main cause of retained placent...

Nuclear actin aggregation is a hallmark of anti-synthetase syndrome-induced dysimmune myopathy.

OBJECTIVE To analyze antisynthetase syndrome-associated myositis by modern myopathologic methods and to define its place in the spectrum of idiopathic inflammatory myopathies (IIMs). METHODS Skeletal muscle biopsies from antisynthetase syndrome-associated myositis and other IIMs from different institutions worldwide were analyzed by histopathology, quantitative PCR, and electron microscopy. ...
